Introduction
In recent years there were widespread perceptions that the delivery of health care service was poor1. The Health Department identified the need for reviewing the health system for greater client benefits and satisfaction.
 In 1998 the Asian Development Bank agreed to provide technical assistance for such an initiative through a grant in aid for defining the extent and scope for reforms needed for improvement in the health care system of the province, in tandem with the provincial governments own reform agenda and in accordance with the Poverty Reduction Strategy Paper (PRSP), mid-term development framework (MTDF) and Millennium development Goals (MDGs). HSRU was established in early 2002 it was the fist unit to be established in Pakistan. HSRU was established in the Health Department with a view to prioritize the reform initiatives, harmonize the donor support, provide technical support to the districts and coordinate human resource development according to the needs of the organization. After Devolution of powers, major problems were poor planning, implementation and capacity to deliver health services.
The proposed four units of HSRU were:
1.         Policy Formulation Unit.
2.         Human Resource & organizational andre- structuring unit
3.         Districts/Institutional Technical assistance Unit
4.         Donors Coordination Unit
